| Cj's Happytails Breeder Of AKC registered dogs Lancaster, Ohio 43130 (614-205-2379) Date____________ Sex____________ D.O.B.____________ Color/Markings___________________________ Litter#________________________ Purchase Price $__________________ Your Puppy Has Received The Following Shots: Canine Distemper,Adenovirus Type 2 Parvovirus, Parvovirus,Canine Coronvirus,Parainfluenza-Bordetella Date Given:____________ Your Puppy Was Wormed With Strogid-T 4 weeks,6 weeks,8 weeks I Cathi Jago am saying that to the best of my knowledge the puppy you are purchasing is guaranteed in good health at the time of sale. It has received temporary inoculations against the most common puppy illnesses as indicated above. I have taken great care to eliminate parasites such as intestinal worms and coccidiosis. Since these conditions are common among puppies I cannot guarantee this puppy to be totally parasite free. Puppies are also known from time to time to have hernias, this is easily repaired when they are spayed/neutered and are not life threatening condition that will not constitute a return or replacement puppy. The purchaser has 72 hrs from the time of sale to have the puppy vet checked to validate this contract, if the purchaser does not choose to have the puppy vet checked this releases the seller of all responsibility and of all obligations to this contract.......If the veterinarian finds a life threatening condition the puppy will be exchanged for another puppy of the same breed of your choice or a complete refund will be given. If the buyer chooses to keep the puppy the seller is released of all obligations for any vet bills. A statement from the veterinarian on the veterinarians heading paper will be required in a case of a return. The breeder is not in anyway responsible for any cost of any veterinarian check-ups,medications or procedures after the sale. This puppy is sold as a pet quality puppy only! And for any reason within the dogs life the new owner cannot house or can no longer provide a home for the above said puppy the new owner is under agreement to return the puppy listed above with all AKC papers back to the breeder (Cathi Jago) this is a (NO BUY BACK AGREEMENT) and (NO REFUND WILL BE GIVEN). This agreement constitutes a final sale unless otherwise stated. I have read, understand and agree to all terms of this contract.
